Check In Process
Admission to the Madison Community Hospital is usually prearranged by your physician. Check-in time on the day of your arrival is dependent upon your procedure and is determined by your physician. Emergency patients are admitted at any hour of the day.
When you are admitted, please bring identification as well as insurance information. You will have an identification band placed on your wrist that tells the hospital staff your name, hospital number and physician's name. You should not remove this band during your stay.
Pre-Authorization / Verification of Insurance
Health insurance companies may require you to contact them before your hospital stay so they can approve your stay in advance. Please read your insurance card to identify the appropriate telephone number to contact. Please bring any insurance cards, or medical coupons if covered by a state Medicaid program, with you on the day of admission.
Copays and deductibles may be requested at the time of service.

- Medical Insurance and photo ID and co-payment
- A list of all medications you are currently taking, including dosage information
- Eye drops that may have been prescribed for you by your doctor
- Nightclothes, robe and sturdy slippers
- Hairbrush, shaving items and other toiletries
- Personal comfort items
- Electrical appliances
- Jewelry and other valuables: Please leave all jewelry, money and other valuables at
home. If you do have valuable items with you, including medications, you will be
asked to seal them in a special envelope which will be placed in the Hospital’s safe. A receipt for the contents will be placed with your chart. At the time of your discharge, please obtain your valuables by presenting your receipt at the reception
area. Keep only enough money with you to purchase newspapers and small items.
